Comparative Study of Living and Cadaveric Kidney Transplantation

Abstract
From June 1989 to August 1994, 211 kidney transplantations were performed in our surgery department, of which 128 cases were from living related donors (60.7%), 42 cases were from living urelated donors(19.9%), and 41 cases from cadaveric
donors(19.4%).
Patients and graft survival rates were 96.8% and 93.3%(lyr) in living donor transplants, and 94.4% and 81.7%(lyr) in cadaveric renal transplantation.
In therms of factors affecting graft survival, postoperative serum creatinine levels (day 1, day f discharge and one month) and presence of a rejection episode were significant for graft survival in both living and cadaveric donors. In addition,
donor's
age, donor's general condition and cold ischemic time were other important factors in cadaveric kidney transplantation.
Pulmonary complications were the most frequent cause of patient death, and rejection episodes were the most frequent cause of graft loss. Causes of graft failure were graft loss itself in 26 cases(12.3%) and patient death in 8 cases(3.8%).
We conclude that for the activation of a kidney transplantation program, increase of cadaveric donor harvest and improvement of initial graft function in cadaveric kidney transplants should be mandatory.
